It's definitely a skeleton issue causing your problem. Honestly, I wouldn't recommend using Deadly Mutilation in it's current state. Using it on a big mod list can cause weirdness like mutilations happening much later than they're supposed to. If you still want to try it out, I think XP32 is supposed to have an option for deadly mutilation.Just so everyone knows, this works just fine with SE/AE as long as you don't actually port over the main plugin or use the 1.3.4 update file. I've been using the original 1.3.3 main files and so far the mod is working as intended.
